Photos: 4 recent photographs of each person on the account. Each person should write their full name and sign on the back of their own photos.

Identification: A photocopy of the National ID (NID) or passport for each person on the account.

Bank Statement: A recent bank statement (from the last six months) of the first account holder.

Tax Information: : A photocopy of the Tax Identification Number (TIN) certificate for both account holders.

Signatures: Both account holders need to sign in the required places in the BO opening book and on the Know Your Customer (KYC) form.

Bank Details: A photocopy of a page from each person’s cheque book showing their bank account details.

For the Nominee:

Photos: 2 photographs of the nominee. The nominee should sign on the back, and both account holders should also sign there.

Identification: A photocopy of the nominee’s National ID (NID) or passport.

If You're Adding an Authorized Person:

Account Book Details: Write the authorized person’s name, signature, and address on the first page of the account opening book.

Photos: 2 photographs of the authorized person, signed by them. The account holder should also sign on the back of these photos.

If You're Setting Up a Power of Attorney:

Account Book: Fill out pages 7 and 8 of the account opening book.

Photos: 3 photographs of the power of attorney holder, signed by them. The account holder should also sign on the back of these photos.

Company Signatures: Sign in the specified spaces with the company seal.

Authorized Signatories: Provide a list of authorized signatories to operate the account and four attested photographs of each authorized person.

Legal Documents: Include the board resolution, memorandum of articles, certificate of incorporation, TIN certificate, and an updated trade license.

For Partnerships/Sole Proprietorships:

Photos: Submit 4 recent photographs of the account holder. Write your full name and signature on the back of each photo.

Nominee's Photos: Provide 2 photographs of the nominee. The nominee should sign on the back, and the account holder should also sign there.

Identification: Include a photocopy of the National ID (NID) or passport for both the account holder and the nominee.

Bank Statement: Attach a recent bank statement (from the last six months) of the account holder.

Tax Information: Include a photocopy of the Tax Identification Number (TIN) certificate.

Introducer Information: Include the name, client code, and signature of your introducer.

Service Agreement: Provide a service agreement with your employer or a trade license.

Salary Details: Include your salary statement, tax pay slip, or Tax Identification Number (TIN) certificate.

For the Nominee (if applicable):

Photos: Include 2 photographs of the nominee. The nominee should sign on the back, and you should also sign there.

Passport & Stamp-sized Photo: Include one passport-sized and one stamp-sized photograph of the nominee.

Bank Confirmation: Provide a copy of a bank statement or certificate from the concerned bank of the account holder.
